A company is looking for a Care Manager RN.
Key Responsibilities
Evaluate member needs and develop personalized care plans
Coordinate care management interventions and facilitate access to
services
Monitor member progress and update care plans as necessary
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Pediatric experience is required
Ability to perform field visits and remote work
