A company is looking for a Care Planner Behavioral Health.
Key Responsibilities
Provide comprehensive support to behavioral health clinical teams
to enhance member engagement and care delivery
Conduct outreach to educate members about specialty programs and
assist them in locating services
Collect information on patient progress and assist with
administrative duties related to care transitions and
authorizations
Required Qualifications
2 years of experience in Behavioral Health care
Previous customer service experience
Proficiency with computer skills, including navigating multiple
systems
Associates Degree or equivalent combination of education and
experience
