A company is looking for a Care Planner Behavioral Health.
Key Responsibilities
Provide support to behavioral health clinical teams to enhance
member engagement and care delivery
Conduct outreach to educate members about specialty programs and
assist with service location
Collect patient progress information and provide details on mental
health benefits and resources
Required Qualifications
2 years of Behavioral Health care experience
Previous Customer Service experience
Proficiency with computer skills, including navigating multiple
systems
Associates Degree or equivalent combination of education and
experience
